Incident Register - Document Class: Index

Incident Log

Fourteen incidents have been registered since NeuroCrypt began keeping a register. Eleven are sealed. Three are served in this extract because their findings bear on published program documentation.

An incident is any event for which NeuroCrypt's own explanation did not survive review. A small number of other matters are registered here because no other register is appropriate to them; requests to construct a deletion path, directed here under NC-PR-0002 §2.1, are the standing example. Equipment failures, injuries, and containment events are handled elsewhere and do not appear here.

On row 0014 Incident 0014 was sealed before it was registered. The register records the seal, the sealing authority, and the timestamp. It does not record the subject, and the office that sealed it no longer exists. The entry is retained because the register is append-only and the row cannot be withdrawn.
