Incident 0001

During excavation of the primary shaft, survey crews placed and photographed numbered reference markers at 20 m intervals along the advancing face. The photographic record is complete and has been continuously in Program custody. The plate series is held under annex; it is not served in this extract.

The marker numbering in the earliest surviving plate does not begin at 1.

Markers in Plate 1 are numbered from № 114. Crew logs for the corresponding shift record the placement of markers 114 through 121, described in the log as "continuing." Every subsequent plate and log is internally consistent from that point forward.

At 20 m spacing, markers 1 through 113 account for approximately 2.26 km of prior survey. That figure is chainage along a surveyed face and is not a depth; the primary shaft is 1,340 m at the controlled figure. No such survey appears in the works program, the excavation contract, the geological report, or the sponsor's disbursement record. NeuroCrypt has no record of a prior excavation. NeuroCrypt has no record of commissioning one.

Review 1-4. Clerical error in the survey crew's numbering. Rejected at Review 5: the numbering is consistent across four independent crews, three plate series, and two instrument sets.

Review 5-7. Markers were inherited from an unrelated prior civil works on the site, not commissioned by NeuroCrypt. Rejected at Review 8: the marker pattern, spacing, and reference datum match Program standard, which did not exist before NeuroCrypt.

Review 9. No finding. The reviewing officer recorded that the file contains no explanation consistent with all of its own contents, and recommended the incident remain open rather than be sealed with a finding the record does not support. The recommendation was accepted.
